Use only the current form supplied with your assigned requirement. ## What your assigned requirement may ask for [#what-your-assigned-requirement-may-ask-for] * The correct form version for your institution, program, and placement period * Your name, student number, program, and other requested identifiers * Every required acknowledgement, checkbox, and initial * Every required signature and date * All pages in their original order Do not copy answers or signatures from another person's form. If a statement is unclear or does not apply to your circumstances, ask the office that issued the form before signing it. ## Current guidance [#current-guidance] Combined acknowledgement forms are created by institutions or placement organizations to collect several acknowledgements in one document. Their subjects, wording, signatories, and renewal periods vary. A similarly named form from another school or placement cannot establish what your issuer requires.
