## What your assigned requirement may ask for [#what-your-assigned-requirement-may-ask-for]

* The current declaration supplied by your program or training institution
* Your name, student or program details, and placement information requested
* Every acknowledgement, initial, signature, and date required by the form
* Every page, including instructions when the requirement asks for them

Read the declaration before signing. Ask the responsible program office about
any statement you do not understand or any field that does not match your
placement.

## Current guidance [#current-guidance]

Ontario provides WSIB-funded or private insurance coverage for eligible
students in approved postsecondary programs during qualifying unpaid work
placements. Eligibility depends on the program, placement, authorization, and
location described in the provincial guidance. Coverage generally runs from
the placement start date through the end date.

Training institutions and placement employers have separate responsibilities.
A signed declaration records the student's acknowledgement of the conditions;
it does not by itself establish that every placement is eligible for coverage.

period, and resubmission rule shown on your assigned requirement control.
