Preparing Clinical Placement Documents for a Smooth Review
Prepare clear, complete, and correctly matched evidence for clinical placement requirements, then follow each submission through review in CareCard.

Understand what the requirement is asking for
The CareCard knowledge guide helps clinical placement students and employees understand the health, safety, and legal requirements they need to complete. Each requirement explains what it is, what to submit, which proof is accepted, common issues to avoid, and how long the evidence remains valid.
Read the specific requirement before choosing a file because acceptable evidence can vary. Immunization and serology requirements, for example, may call for a vaccination record, a laboratory report, or both. CareCard lists two documented MMR doses or a blood test showing immunity as accepted pathways, while hepatitis B requires the vaccine series and a blood test confirming immunity. If a requirement is unclear or may not apply, confirm it with your program before submitting.
Make identity and dates easy to confirm
Upload the complete document with every page and all four corners visible. Keep names, dates, signatures, stamps, and results sharp enough to read at normal viewing size. Cropped edges, missing pages, glare, shadows, and blur can prevent a reviewer from confirming otherwise valid information.
Your full name must appear on the evidence and match the name your program has on file. The relevant issue, expiry, vaccination, or test date must also be visible and current for the requirement. A nickname, initials, an unexplained former name, or a missing date may require clarification before validation can be completed.
Prepare one review-ready file per requirement
CareCard accepts PDF, JPG, JPEG, PNG, HEIC, HEIF, TIF, TIFF, GIF, and WEBP files up to 25 MB per document. Combine a multi-page record into one file in the correct page order, and do not upload a password-protected or edited version. Open the finished file on your device before attaching it to confirm that it loads as expected.
When you upload from a requirement page, CareCard automatically names the file after that requirement. When you upload through My Files, choose a descriptive name, such as "HepB-Serology-2026.pdf," so the document is easy to recognize. If the file is already stored in My Files, use Attach Existing to connect it to the appropriate requirement instead of uploading another copy.
Follow the submission through validation
Submitting a requirement locks its attached files while they are being reviewed. If a replacement is needed, cancel the submission, attach the correct evidence, and submit it again. CareCard displays the current status and any reviewer feedback, giving the applicant a clear next step when more information is required.
