Combined Acknowledgement Form

How to prepare the institution-specific combined acknowledgement form assigned by your program or placement.

Applicability: Your institution, program, or placement may assign a combined acknowledgement form.

CareCard catalogue default: 180 days.

Evidence reviewed: August 31, 2026

There is no single Ontario-wide combined acknowledgement form for all placements. Use only the current form supplied with your assigned requirement.

What your assigned requirement may ask for

  • The correct form version for your institution, program, and placement period
  • Your name, student number, program, and other requested identifiers
  • Every required acknowledgement, checkbox, and initial
  • Every required signature and date
  • All pages in their original order

Do not copy answers or signatures from another person's form. If a statement is unclear or does not apply to your circumstances, ask the office that issued the form before signing it.

Current guidance

Combined acknowledgement forms are created by institutions or placement organizations to collect several acknowledgements in one document. Their subjects, wording, signatories, and renewal periods vary. A similarly named form from another school or placement cannot establish what your issuer requires.

The current assigned form and the instructions from its issuing organization are the authoritative sources for completion.

CareCard validity

CareCard's current catalogue default is 180 days. This is a tracking period, not a universal legal lifetime for acknowledgement forms. The version and date rules shown on your assigned requirement control.
