Health Status Communicable Diseases Declaration

How to prepare a communicable-diseases health-status declaration when a paramedic program assigns this requirement.

Applicability: CareCard's current catalogue associates this declaration with second-year Primary Care Paramedic and first-year Advanced Care Paramedic students. Your assigned requirement determines whether it applies.

CareCard catalogue default: 365 days.

Evidence reviewed: August 31, 2026

Use the current declaration supplied by your program. Provide only the information and health professional completion the form requests; do not substitute a general medical note.

What your assigned requirement may ask for

  • The program's current health-status declaration
  • Your name, program, year, and other requested identifiers
  • Every declaration, signature, and date required from you
  • A physician, nurse practitioner, or other authorized professional's section when the form requires one
  • Every page of the completed form

If your health status changes or you are unsure how to answer a health question, follow the form's instructions and consult the program or an appropriate health professional.

Current guidance

Ontario Regulation 257/00 sets communicable-disease and immunization qualifications for emergency medical attendants and paramedics. It also applies specified protections to students who attend or assist on ambulance calls. The incorporated Ambulance Service Communicable Disease Standards list the immunization requirements and diseases from which covered individuals must be free.

The program's declaration is an administrative way to collect evidence for those requirements. The regulation and standard do not create one universal student form.

CareCard validity

CareCard's current catalogue default is 365 days. The applicable academic year, declaration version, and update instruction shown on your assigned requirement control.
